Power of Attorney Form

If a company or third party will handle the pickup on your behalf, you must complete and sign the official power of attorney form.

Two New Photos (for Immigration Visa Pickup)

If your Canada visa is an immigration category, you must include two recent photos with your documents.
Photos must be unaltered, without retouching or digital editing.
